Your first Visit:

Your first visit will take between 50 and 80 minutes depending on the complexity of your case. It is recommended and will be helpful to reduce the amount of time needed to gather information if you have the intake forms filled out and returned to the office at least 2-5 days before your first visit.  Please bring all the supplements and medications you are currently taking with you to the appointment.

During your visit, Dr. Powell will go over your intake information with you, perform a physical exam and go over basic treatment philosophy and guidelines.  He will evaluate whether or not you will need additional lab/blood tests and determine if he will need additional information from your other healthcare professionals.  Dr. Powell will then begin setting up a Personalized Health Plan (PHP) for you.  In some cases, the complete plan cannot be completed this first visit because information from various lab/blood tests or previous and current doctors is required.

You will be scheduled for a follow-up visit. Because the second visit is critical to setting up the PHP, you can get a significant price reduction on the follow-up if you pay for it in advance with your first visit.

Paperwork:

Below you will find the Patient Intake and  Symptom Questionnaire forms.  Most people take 30-45 minutes to complete all the paperwork. If possible, please fill them out and return them 2-5 days before your appointment. Also, please return a Medical Authorization Form for your primary care doctor and each medical practitioner you are seeing for your condition, so we can start getting information from them.  If you are mailing the forms, please allow a couple of extra days to make sure the forms are not delayed for any reason.  The forms may be faxed, scanned and returned by email, dropped off (there is a mail slot for after-hours drop off) or mailed.

  • Payments and Insurance:

    We are not a member of any plan, and are not currently filing your insurance claims. However, a growing number of insurance plans are covering naturopathic medicine.  Check with your plan. We do  provide out-of-network coverage for these plans and will provide a receipt with the appropriate codes for you to get reimbursement from your insurance.  Payments are expected at time of service.  We take checks, Visa, MasterCard, and Discover cards.
